To expedite your first visit, please:
- Bring completed new patient intake packet (see below for download)
- Bring your driver’s license/ID
- Bring your insurance card, if applicable*
*Note: For those patients without insurance (Fee For Service), we offer affordable payment options. Please contact our office to discuss fee schedules with our staff.
Please allot 45 minutes to one hour for your initial visit. After taking a history of your case and general health profile, Dr. Radice will perform a series of orthopedic and neurological tests as well as a physical exam. If deemed necessary, Dr. Radice will order x-rays for you.
